I had an exposure with a sex worker 56 days back, it was protected, however naked oral (blowjob) and kissing was involved. I had noticed red bumps on her hands and red circular rashes on her body. I took Viropil PEP for complete 28 days without missing a dose. After stopping PEP, four to five days later I noticed some bumps on both the corners of my lips and white tongue and sore throat. I checked with doctors and they said this is angular chelilitis. I also had a sore throat ever since. I took two duo tests ELFA at 35 days post exposure and 54 days post exposure and the results were p24 antigen - Not determinable, antibody - 0.12 and 0.11 ( reference range 0.25). I also took elisa duo at 40 days and that was non reactive too (0.11 - range 1 S/CO). I also took tests for HbsAg, HCV, VDRL, TPHA, HSV 1 and 2 IgG, IgM and all negative at 5 weeks. Why is this sore throat, white layer on tongue and bumps on lips happening ? Do I need some other tests like Gonorrhea, Chlamydia?

Dear Patient,
Thank you for sharing your concerns. Based on what youâve described, your risk of HIV from this exposure appears to be very low, especially since you used protection during intercourse and completed the full course of PEP (Post-Exposure Prophylaxis) on time.
The tests youâve done so far (including HIV Duo/ELISA at 35, 40, and 54 days) are highly reliable, and your results have been non-reactive, which is a very good sign. The symptoms you are experiencing now â like sore throat, white coating on the tongue, and bumps at the corners of the lips â are more likely related to a local infection, fungal overgrowth, or dryness, and not necessarily an HIV-related illness.
However, to fully rule out all possibilities and ensure you get complete peace of mind, it would be good to also consider checking for other common STIs like Gonorrhea and Chlamydia, especially since these can sometimes cause throat symptoms if oral contact was involved.
I recommend that you consult with me personally, so I can review everything in detail â including your test reports and current symptoms â and guide you on whether any further tests or treatment are necessary.
Donât worry â youâve already taken the right steps. Letâs just complete the evaluation so you can move forward with confidence.
Take care,
Dr. Amit Kumar Patel
