The recurrence of TOF in subsequent pregnancy with or without a family history is low, it is about 3-5 %. But, this is higher than the chances in the general population without a history of previous pregnancy with TOF (0.8%).
For prevention: If there is diabetes in the mother the blood sugars should be well controlled prior to conception (HbA1C < 6.5%). Must avoid alcohol and smoking. Some medicines taken for epilepsy (fits) can cause heart problems in the baby, if those are taken then they will be changed to safer ones before pregnancy is planned.
