My son 20m old speaks only 4-5 one syllable words(caa for car,baa for ball, bus etc)mispronounces doesn't use bisyllable words, doesn't pay much attention to what I say while trying to teach him.follows only few commands most of the commands he doesn't understand or doesn't listen.he cannot identify his body parts.he doesn't call me mama.. He doesn't have hearing problem for sure..reacts for even smallest sounds and reacts for his fav words like light, cocomelon(rhyme channel)etc.. Doesn't have any oral issues like tongue palette he chews food properly.and can put tongue out. He has good congnitive skills , solves simple puzzles like shape sorting and colour matching.does or imitate daily activities like sweeping , putting clothes in machine, watering plants He points to objects has eye contact has health bonding.blabbers a lot uses sign language and pointing.makes sounds rather than trying to speak.i suspect a speech delay, not sure what is to be done next. Please suggest..im concerned.

Many children especially boys are suspected of speech delay at around 2 years of age.  But only few of these kids have actual speech delay and most of them cover up till their school going age.  Around 5 years.  We expect 20 months old child to speak atleast 10 - 20 words with meaning . Some children are late bloomers or late talkers.  Is there any history of similar speech delay in your childhood or in his mother's childhood ? If yes then no need to worry.  He will pick up .  As per ur description his cognition appears fine and also hearing appears fine.  No signs of autism also as per ur history.
Next Steps
I think we can wait few more months and continue stimulation for speech.  By talking to him and encouraging him to speak.
Health Tips
If he loses any skill he has gained like from bisyllables to monosyllable or not progressing in next 5-6 months then u need to get him evaluated from pediatric neurologist, audiologist , speech therapist if required.

Hello. Great. You have given a good history and covered most of the developmental areas . Still, require birth details and he needs a complete physical evaluation.
Next Steps
Consult online or meet a developmental specialist for extensive evaluation
