This is keratosis of tonsil where the crypts (small spaces within tonsil) are filled with minute food particles and that causes halitosis in long run. The main reason for keratosis of tonsil is Chronic tonsillitis.
Next Steps
Maintain oral hygiene Do salt water or Betadine gurgle
Health Tips
tonsillectomy is the ultimate treatment but it can be avoided doing gurgling. More the gurgling, more whitish material is flushed out from the crypts and that keeps the problem away.

Sir your throat looks grossly inflamed, the whitish material is tonsillar concretions which occurs when there remains chronic infection/inflammation. U need to get your chronic tonsillitis treated. And maintain good oropharyngeal hygiene. Use h2o2 gurgles 1 tsf diluted in half glass of water three times a day.
