Weakness Fatigue

Asked for Male, 34 Years

I had got removed my leg rod a month back and from few days I have full sleepiness,extreme fatigue,shortness of breath,blurry vision sometimes,I have attached my blood report taken before surgery.is any deficiency causing all this symptoms,I suffer from GERD with mild hiatus hernia from few years.I even suffer from frequent UTI.what medication should I take to overcome this problem.is it due to anemia all this happening.
