Hello doctor
I have just got my random blood sugar test done and the range is very high 375. My HBA1C levels are currently at 8.9. I just had c section delivery 3 months back and did not have any high blood sugar issues then. I do not want to take in insulin. What medicines can i take safely to reduce blood sugar while breastfeeding?

Hello,Your blood sugar levels (RBS 375 mg/dL and HbA1c 8.9%) are significantly elevated, which indicates uncontrolled diabetes, likely postpartum onset or previously undiagnosed.
Since you are breastfeeding, certain medications are considered safe:
Metformin is the first-line and safest oral drug during breastfeeding
Glibenclamide (Glyburide) can be considered if additional control is needed
Insulin remains the safest overall option and does not harm the baby, despite concerns
However, with such high sugars, oral drugs alone may not be sufficient initially, and short-term insulin might be required to bring levels under control.
Next Steps
Get fasting and postprandial blood sugar levels done
Check renal function before starting medication
Start treatment promptly under physician supervision
Monitor blood glucose regularly at home
Follow up within 5–7 days for dose adjustment
Health Tips
Do not delay treatment, as persistently high sugars can lead to complications
Maintain a diabetic diet (low simple carbs, high fiber)
Continue breastfeeding—it is beneficial for both mother and baby
Watch for symptoms like excessive thirst, urination, or fatigue
If sugars remain very high or symptoms worsen, insulin should not be avoided as it is both safe and effective.
Take care.
