Uncare behaviour at home and not talking
Hi Doctor, My brother who is 32 years old is showing uncare behaviour at home. He was lastly working In an IT company in Navi Mumbai whereafter due to medical reason was voice being heard in ears and having headaches plus head spining from inside he had to quit his job around 4-5 years back. However he was treated and was told to consult a psychiatrist which we couldnt do now he is sitting at home since 4-5 years without a job. As he is about an age of marriage family is very upset about his attitude of uncareness towards family. He doesn't speak to anyone in family except mother which also he speaks online for food. All the time he is sitting on his personal computer looking at it but whenever we try to see he puts the screen off. Moreover I have observed that he sometimes watch porn videos. He also reads some novel over the phone after vacating he PC and laugh in himself. He doesn't socialize and we are trying to look for a doctor who can visit home and do checkup.

Hello Mr. Good morning. I tried to empathize with ur brother situation,I got an impression,that his thought ,mood , behaviour and personality has altered with passage of time. Moreover, his lifestyle ,mood , behaviour is indicative of "Negative symptoms or depressive symptoms " both symptoms are suggestive of Psychotic or neurotic phenomenon. What is important now ? 1.You brother must be under surveillance of any significant family member. 2.Balanced diet is very important in this case. 3.Avoid argumentation with him on any issue. 4.Before understanding his condition , don't force him into marriage ,as marriage is goin to worsens his symptoms if he is sufferings from psychiatric illness. Unnecessarily his wife will suffer.
Next Steps
Consult a Psychiatrist or Neuropsychiatrist of ur choice  and allow him/her to assess the condition of patient in much more details. Evaluation of present, past , family and personal history is very important in this case. Mental health examination of patient will be very helpful.

According to the description given by you, he must be suffering from schizophrenia, what he is exhibiting is negative symptoms (not doing anything/socializing) He needs medications to control his symptoms later most importantly rehabilitation.
