Toothache in the lower last rightside.
I had temproray filling by 1 dec and permanent by 3 dec. later i felt pain after permanent filling. thinking that its normal i dint pay much attention. later at night i felt like i get shock in my ear or brain, as it the right last and down tooth so i still ignored the pain. yesterday my pain got severe and i went to private dentist she checked and told me that there is a milk tooth like just a tip surrounded by whole gum so you can extract it. later she told give me antibiotics. what to do next

I am guessing u r talking about wisdom tooth of which only a small cusp(tip) is visible. Yes if the position and angulation of tooth is no favorable go for extraction.
