Hi Tooth aches usually have an underlying cause which requires professional intervention of some sort. extraction is not necessarily the only option for treatment of toothache. The actual treatment depends upon the actual diagnosis of the underlying cause. However, delay in seeking professional opinion and care may allow the ongoing problem to worsen and necessitate more invasive options to be employed, including extraction.
Next Steps
Set up an appointment with your dentist at the earliest to allow institution of appropriate therapy as early as possible.
Health Tips
In the meantime, while you're waiting for the appointment, do rinse your mouth with lukewarm salt water as many times a day as possible. You can use some over-the-counter pain medicines as needed. However, do not start antibiotics without first seeing a dentist.

It suggests me that there is a root canal failure. Plz visit the dentist for a clear X-ray . Re root canal can be done if possible. Extraction is the last resort.
Next Steps
