More description is needed and a dental visit is required to assess the condition if pain is related to the tooth. Cavities can be internal also and can be diagnosed by a x-ray only. And only a dental visit can clear if its a dental pain or other than that.
Next Steps
visit to a dentist and get an xray done

hello, you need to visit a dentist and get it checked. get an x-ray done. if there is a cavity, get it filled. if there is infection to the root of the tooth, get it treated. then get a crown placed over it.

Dental caries can be present in hard to see places e.g. Between two teeth. A good xray will dictate further treatment

Even if you dont see any cavity, there might be many other reasons for tooth ache. Like gum infection (periodontitis), discoloration of the tooth, wisdom tooth impaction(if its last tooth in the arch) So i would suggest you to consult a dentist and find out the exact reason n treat accordingly.

A logical opinion cannot be made based on the description that you have mentioned..... your friend needs to visit a dentist for a proper clinical examination and an x-ray if required...... cavities in the teeth might not always be directly visible to the naked eye....
Health Tips
do not delay the treatment because if not treated, the symptoms will aggravate in the future.....

The pain could be either due to caries in areas that are difficult to see or development of abscess due to a fracture due to trauma or fracture of tooth. A good dental examination and X ray is the only way to know. Start with a dental visit
