If you really want to stop smoking and alcohol, you should and would stop it.
There'll be a price to pay but if you have the conviction, u won't need any medical intervention in order to get over it.
Although, if ur alcohol history is chronic, u might need medical attention
But not for Smoking.

Stop smoking within a month. Replace your cigarettes with the nicotine lozenges. And stay prepared for the craving. If u can't handle your craving, it means u r not yet totally willing to leave smoking
