Thought process

Most of time, I am able to find the root cause of my problems. However, I am not able to follow steps. Because of my current situation and my past failures, I feel mentally stretched. I try to do meditation to resolve them but not able to do meditation as well. Also while speaking, I face headache.

The symptoms as you mention could be manifestation of stress, anxiety, or a depression. Leading a healthy lifestyle, exercise, meditation, yoga could help you. Pursuing a hobby, taking time out with family and friends is also likely to help you. Writing a diary may help you express your thoughts better. In case the problems persist or you are having other distressing symptoms seek an in person consultation with a psychiatrist or a clinical psychologist to help you. Hope this helps you..
