Testicular pain. What should i do. i am?

I have swelling in my left scortum since from 8 months, i have taken many antibiotics but none of them cured my scortum pain i have pain in my both testicles Imortant i have to say something- Before 8 mnths i used to mastribute but at the end when sperms comes out i hold my penis so sperms cant come out. i did it for 5-6 days after then i have this problem someone help me what should i do. My both spermatic cord dilated. I have bilateral varicocele. I have attached my colour doppler of my scortum first report of 16 march 2017 second report or november 2018 Please check my reports and tell me what should i do. I am thankful please someone say me what to do i am 19 years old only help me please
