It can be testicular atrophy due to infection or harmonal changes. It needs to be diagnosed properly and then the right treatment should be started. Get your USG - Testis done once. It can be well treated with homeopathic medicine effectively and without any side effects. You need an expert Sexologist who is a good homeopathic physician.

A new perceived reduction in the size of one testicle, particularly when associated with episodes of pain, should not be ignored. There are several possible causes, including previous inflammation/infection, varicocele, hormonal or vascular problems, and less commonly testicular atrophy. The reduction in morning erections may be related or may be an independent issue; it cannot be used to determine whether the testicle is shrinking. Any new change in testicular size, shape or consistency warrants an examination.
Next Steps
I would recommend an examination, rather than waiting to see whether the size changes further. Depending on the examination, a scrotal ultrasound with Doppler may be useful to objectively compare both testes and assess blood flow, varicocele or other abnormalities. Do not assume that surgery or any particular treatment is required until the actual cause is established. For better treatment and personalised guidance, you may connect with us at 9 3 1 5 6 6 1 5 6 5 for proper assessment and guidance.
Health Tips
Avoid repeatedly squeezing or measuring the testicle, as this can increase anxiety and cause local tenderness. Wear comfortable supportive underwear and avoid significant trauma to the area until you have been evaluated. If you develop sudden or severe testicular pain, swelling, nausea/vomiting, redness, or a testicle that suddenly sits higher than usual, seek emergency medical attention immediately, because testicular torsion is a time-sensitive emergency. Thank you

Right testicular shrinkage with intermittent pain and reduced morning erections needs urgent evaluation. This could indicate a past torsion episode, varicocele, epididymo-orchitis, or hormonal issue — all of which require proper assessment.
Next Steps
See a urologist soon and get a scrotal Doppler ultrasound to assess blood flow and testicular size. Don't delay as some causes can affect fertility. Consult me on Practo for further guidance.

change in testicular size with a history of pain should not be ignored. It may be due to normal testicular retraction, but testicular torsion or other conditions need to be ruled out.
