The fasting is high but, with in acceptable range for extraction. PP is not, even if the extraction is done with maintaining good oral hygiene, implant placement is not a good idea due to delayed healing associated with high Blood sugar which tends to increase with any kind of infection.
Next Steps
My advice, get your Bl sugar in Control before proceeding.
Health Tips
Diet modification with cardiovascular workout will surely do the trick for you in consultation with your physician.

Your blood sugar levels do seem high . Are you on medication ?
Bringing the blood sugar levels closer to normal levels with medication n regular exercise will help you go through your dental treatment smoothly . Higher blood sugar levels will delay healing of wounds n increase chances of infection. So yes may b a visit to your diabetologist before treatment will b helpful.
However if for some reason the extraction cannot wait - then make sure the doctor checks your blood sugar levels before the extraction. From your end - you need to keep the wounds really clean ,rinse your mouth every time you eat something, in order to prevent any infections . All the best n take care :)
Health Tips
After firat 24hrs of extraction , gargle your mouth at least 3-4 times a day with warm water n salt solution , to keep the wounds clean.
You can proceed with extractions with the present values, yes definetly sugar levels matter for both extractions as well as implants ( healing gets delayed & post pp infection)
