Only increase in size or presence or absence of pain not criterion to conclude about cancer. Since lymph nodes drain a area, that also comes to account. On the whole infectious nodes are more common than cancer.

Not all the swollen Lymphnodes are cancerous.
Painful lymph node mostly related to infection .
