Sore size increased

Hello Doctor, My mother in law has a sore which was in red colour and it was like a bubble and it has been present since last 5 years and now all of us sudden the same bubble size has been increased ( P.F.A the image FYR) Can you tell us what is this and what are the next step? She is 58 years and reducing weight gradually and she has sugar , pressure issues.. Thanks
