Skin rashes and itching

Asked for Male, 29 Years

I get some sort of skin reaction and uncomfortable, irritating sensation that creates an urge to scratch and I can't control it. This mostly happens on my hands, and face. Sometimes on legs as well. The part of my skin gets swellen if I scratch. Skin on my hands and thighs has become very rough because of this. I have no idea what is causing this, but I am really tired and want a permanent solution.
