Whole-body itching in the third trimester that focuses on the hands, feet, legs, and abdomen can be obstetric cholestasis of pregnancy. This is a treatable condition, but it does need to be checked for, because if it is present the baby needs closer monitoring in the last weeks. Not every case of itching in pregnancy is cholestasis (stretched skin, hormonal changes, and heat can all cause itching), but at 9 months the bile acid check is worth doing.
Next Steps
Please book an appointment with your obstetrician within a day or two and ask specifically for a bile acid blood test. If the level is above 10 to 19 µmol/L, treatment usually starts with ursodeoxycholic acid, and monitoring of the baby is stepped up. If the level is normal, simple moisturisers and cool baths usually help.
Health Tips
Please avoid using strong steroid creams or antihistamines on your own to control the itch. If cholestasis is present, these mask the sign without treating the cause, and monitoring the baby is the more important half of the plan.
