Asked for Male, 28 Years

Hi, Since 5 years , I have black patch of skin on my ear lobes and knuckles. I have contacted several dermatologist no one is able to give me a visible results. Taking vitamin b12 and D from few months. Also applying kozilite cream everyday. No improvement. Please see attached photo.
