  4. Sinus problem. A bad odour as well!

I have been having sinus problem for more than 3 months. now i have started having severe headaches in last couple of weeks. phlegm (when i spit) comes in greenish color. I could sense a bad odour as well

As per your description you might have developed chronic rhinosinusitis.. need antibiotics and other supportive treatment.. visit an ENT specialist for proper assessment..
