If the child is doing goi in other areas that means he has norma IQ level,You can visit a psychologist to get him assess for this problem ,You can opt for one to one sessions for your child

And if possible get his Learning Style tested. We all have different learning styles and if we are taught in that style we learn better and easy.
Puberty (12 years old) is a difficult phase for most kids because their body, mind, and social experiences are changing rapidly and sometimes they face issues adjusting to this change, which may impact their academic performance as well.
Apart from this there might be other underlying issues which need to checked by a Psychologist. Do take minimum 4-5 sessions to understand his condition, as kids take time to develop trust before sharing their thoughts and emotions.

It is very nice to know that he is trying to work hard and improve himself in studies. If we look at the grades, then it can have many reasons. You should be very happy for his hard work. Maybe he is having some problems with the study pattern or with something that you aren't able to identify.
