Shoulder pain could be due to issue with Bursae( fluid filled pocket in shoulder joints) ,muscles or shoulder joint itself . AS patient do experience some peripheral joint issue and that need to be ruled out by physical exam and investigations. In some cases shoulder pain could be due to referred  pain caused by issues going on with internal organs such as heart and it also needs to rule out.
