Sexual Penetration Performance Issue
I’ve had one previous sexual experience where I wasn’t able to perform properly, and the relationship ended. Since then, I’ve become very conscious about sexual performance. Even with my current partner, where there is no pressure for penetration and we’re comfortable with other forms of intimacy, I frequently start worrying that I may not be able to maintain an erection or consummate. I then find myself monitoring my erection and worrying about failing. More generally, I’ve noticed that although I can often get an erection, I sometimes have difficulty maintaining it and it may gradually go away. This has happened repeatedly, so while I suspect performance anxiety may be contributing, I’m concerned there could also be a physical factor. I’d like to understand whether this is primarily psychological, physical, or a combination, and whether I need any evaluation or treatment.

From the history you describe, performance anxiety appears to be a significant contributor, particularly because the problem began after one distressing sexual experience and is now accompanied by anticipatory thoughts such as “What if I cannot maintain my erection?” and repeated monitoring of erection firmness. This can create a cycle of anxiety → increased attention to the erection → reduced arousal → loss of erection → greater fear during the next encounter. However, it would not be appropriate to label it purely psychological without assessment. Erectile difficulties are often multifactorial, and physical and psychological factors can coexist.
Next Steps
A proper sexual-health evaluation should assess your erection pattern during partnered sex, masturbation and sleep, along with relevant medical, lifestyle and psychological factors. If your clinical history suggests it, appropriate evaluation can rule out significant physical contributors. Importantly, psychosexual counselling/sex therapy can be highly useful for performance anxiety, particularly when combined with appropriate medical management when required. Current urological guidance specifically supports addressing performance anxiety and involving a mental-health/sexual-health professional when psychological factors are contributing. Health Tips
With your current partner, try to temporarily remove penetration and erection firmness as the goal of intimacy. Focus on touch, kissing, stimulation and pleasure without checking whether your erection is “hard enough.” If you notice yourself monitoring the erection, gently shift your attention back to the sensations and connection with your partner. Avoid repeatedly testing yourself through masturbation or intercourse to prove that your erection works. Adequate sleep, regular exercise, limiting alcohol/smoking and managing general stress can also support erectile function. This pattern is very treatable, and one previous unsuccessful sexual experience does not predict your future sexual performance. Thank you
