A swollen structure on thumb joints which is very painful ..consulting a physiotherapist at present but is not very helpful.

This could be a Dupuytren's contracture or trigger thumb. Try: Hot water compression and massage. Intralesional steroid injection offer great help. Hand exercises to keep joints moving.
