Just now this is in shape of small grapes which discharge some fresh blood last night may be it was rubbed against the floor. I cleaned it and applied kiskin lotion. It might be a localized skin infection or folliculitis, continue with kiskin lotion and if no improvement then better to consult a vet. Do not allow him to lick or rub it against something as it will not heal otherwise

Dogs scrotal skin is more sensitive than the other skin. As a result, whenever there is any irritant or allergic material come in contact with the skin, it will have reactions like rashes or piples, etc. It will get cured with medicines. In your pet's case, without seeing the issue, it is very difficult to say what it looks like.
Scrotal tissue being a sensitive dependent part may develop some itching as it touches ground quite often. Your dog's pimple type scrotal growth may be due to contact dermatitis, physical trauma, bacterial or fungal infection.
Next Steps
Licking scrotal tissue aggregates it and makes it perpetual. Try to avoid licking , so as to keep it dry, with the help of E-collar if it's so.
Health Tips
See your vet once again. It's a simple problem to resolve in the beginning. Delay in treatment and constant licking makes it worse. Hope this helps. thanks
