The pillars and the mucosa over tonsils look a little inflamed. Since it’s troubling you from last 2 years, would suggest getting checked for LPRD(laryngopharyngeal reflux disease) which is caused due to acidity and can lead to such changes. Tonsillar inflammation during acute infection may present as ear discomfort due to referred otalgia.

The right tonsil being slightly bigger for 2 years is usually not serious but should be checked to be safe. The ear tightness and itching may be due to long-term inflammation like chronic otitis externa. An ENT visit with a simple scope and maybe a scan can help find the cause.

Tonsil size could be related to chronic inflammation. Ear pain may lead to otalgia.
