What is the age of the patient? How long he has been suffering?
The first report is suggestive of subacute intestinal obstruction. May be after an episode of acute appendicitis which might have ruptured and made into an infected mass.
He should be on nil orally and investigations done for pre-surgical profile.
Second report is ultrasound scan. Not satisfactory due to poor preparation of the patient.
