So your query is 'What is meant by lower rectum is opacified?' Normally our rectum is radiolucent as xray can pass through it. In fistulogram a special dye in used through which xray cannot pass just like our bone & comes white. So, some dye is there in your rectum. You have no need to worry about the opacity just visit a surgeon for that fistula. Thats all. Stay healthy.

It must be a simple fistula measuring 6.75cms needs fistulectomy. Lower rectum must be opacified because of the dye injected.
