My mother had mital valve repair in 2009 and left above knee amputation in 2021 Jan and she is taking below medicine and No BP,No diabetic
1-Rivaroxaban 20mg 1dose 10am
2- Ecosprin 150/20 1 dose 10pm
3- Betaloc 25mg - 2 dose 10am/10pm
I have three questions-
1- Shall we continue these medicine only or any modification required?
2- As we consulted few cardiologist some suggest Warf should be given not Rivaroxaban why?
3- Do we need to discontinue any above medicine for some days for vaccination?

Continuing Riviraxoban is fine
No need to stop any blood thinners before vaccination
Using warfarin is for mechanical prosthesis and significant mitral stenosis;

Anticoagulation after Mitral valve repair is required if patient is in Atrial fibrillation (more than three months after surgery)
warfarin can be taken instead of the Rivoraxaban but you need to monitor PTINR regularly
vaccine can be taken we need to Rivoraxaban for couple of days before taking vaccine

Anticoagulation after Mitral valve repair is required for 3 months . Then it can be stopped and antiplatelets like Ecosprin can be continued.
However if the patient has other indications for anticoagulation like Atrial Fibrillation, Rivaroxaban needs to be continued.
Can take vaccines while taking these tablets.
