My mother Aged around 58 was suffering from fever since last 1.5 months and initially her blood test shown infection in her blood and urine culture etc. Was normal. With antibiotics her infection has gone but still she was suffering from fever atleast for few hrs. In a day. Last week she started facing pain in one side of her back and her blood test reflected increase in urea and creatinine level. Her ultrasound, CT Scan and blood test reflected problem of PUS in one kidney and she was put on medicine but she was not getting any relief initially infact with medicine immediately she is suffering from nausea and abdomen pain. But after few days she become stable to some extent with ongoing antibiotics medicines course but now she has loss food appetite and she is not able to digest food. She has to vomit to get relief and she has lost energy, just lying on bed. Her condition is not improving despite of medication. Kindly guide me for future the course of action.

Your mother is suffering from renal abscess/pyelonephritis ( need to see the CT scan report for confirmation). This can occur in old age patients and people with comorbities like Diabetes, prostate enlargement or any urine tract obstruction like stones. If it is an abscess then it will need an antibiotic cover for atleast 4-6 weeks. Followed by repeat scan and urine culture tests before stopping antibiotics. And symptoms she has is related to gastritis which can occur with long duration of antibiotic and illness.
Next Steps
1.First to improve her diet and control gastritis, we need to start PPI tablets. 2.Complete the complete duration of antibiotics. 3.Share your complete scan report with a Nephrologist so as to guide you further. 4.Keep a check on Creatinine and daily urine output.
Health Tips
Check HbA1c rather than random sugar in patients with raised creatine to diagnose diabetes.
