Premature Ejaculation
I have had Phimosis since my childhood. I felt itching in my penis when I was about 12-13 and due to that I used to rub over the penis. As I grow up that habit is converted into masturbation. Since then I used to masturbate but the habit and frequency increased with time. Now I am 21 years old. I masturbate every 3rd or 4th (sometimes more or continue 1-2 days or 2 times a day). My Ejaculation time has become only about 10 to 30 seconds only. What should I do now ? I have tried so many times to quit this , tried Yoga , exercise but nothing worked. What should I do ? Should I take any types of medicines to cure it ?

Based on what you've described, there are three separate issues: Phimosis (tight foreskin) Premature ejaculation (ejaculating within 10–30 seconds) Difficulty controlling masturbation These are common problems and can be treated. They are not a sign that you have permanently damaged yourself. 1. Phimosis Since you've had a tight foreskin since childhood, you should be examined by a urologist. Treatment depends on the severity: Mild cases often improve with topical steroid cream (prescribed by a doctor) along with gentle stretching exercises. More severe or scarred phimosis may require a minor procedure such as preputioplasty or circumcision. Treating phimosis can also reduce discomfort and improve sexual function. 2. Premature ejaculation Ejaculating within 10–30 seconds suggests premature ejaculation, but this can improve. Treatment usually includes: Behavioral techniques such as the start-stop and squeeze methods. Pelvic floor (Kegel) exercises. If needed, a doctor may prescribe medications such as dapoxetine or certain SSRIs, or recommend topical anesthetic creams/sprays. These medicines should be used only under medical supervision. 3. Masturbation habit Your current frequency (every 3–4 days, with occasional more frequent episodes) is not necessarily excessive. The concern is that you feel unable to control it, and it causes distress. Instead of trying to stop completely: Reduce exposure to pornography if you use it. Identify your triggers (boredom, stress, being alone). Keep yourself engaged with study, work, sports, or hobbies. Avoid lying in bed with your phone late at night, which is a common trigger.

Health Tips
Regular exercise, adequate sleep, and stress management improve sexual health. Avoid repeatedly testing your ejaculation time during masturbation, as this often increases anxiety. Remember that masturbation and intercourse are different; many men who ejaculate quickly during masturbation perform better during partnered sex. The good news Your condition is treatable. At 21 years of age, there is every reason to expect improvement with proper management. Thank you

First, please be reassured: you have not permanently damaged your body or your nervous system. What you are experiencing—rapid ejaculation during solitary masturbation in the setting of long-standing phimosis—is a very common, highly treatable situation caused by two distinct factors: Phimosis and Glans Hypersensitivity: A tight foreskin prevents the glans (head of the penis) from naturally retracting and becoming desensitized through everyday contact. As a result, the penile nerves remain extremely sensitive to tactile friction, lowering your physical ejaculatory threshold. Conditioned Fast-Ejaculation Habit: When masturbation is practiced repeatedly with the goal of quick release or driven by guilt, the brain and pelvic floor muscles undergo "conditioned learning." You have essentially trained your autonomic nervous system to trigger the ejaculatory reflex rapidly upon stimulation. Masturbating every 3 to 4 days (or occasionally twice a day) at age 21 is a completely normal biological frequency, not a harmful "addiction." Attempting to forcefully quit masturbation creates a cycle of frustration and guilt, which increases anxiety and makes ejaculatory control even harder.
Next Steps
Consult a Urologist In-Person for Phimosis Assessment: Have a urologist examine your foreskin tightness. Treating the underlying phimosis—whether conservatively with a short course of topical prescription steroid cream (e.g., Betamethasone) alongside gentle stretching exercises, or through surgical procedures like frenuloplasty or circumcision—allows the glans to gradually desensitize and improves ejaculatory latency. Retrain Your Ejaculatory Reflex (Behavioral Sex Therapy): Instead of trying to stop masturbating, change how you masturbate: The "Stop-Start" Method: When you feel yourself reaching 70- 80% of your climax threshold, stop all physical stimulation completely, let your arousal level drop, and then resume slowly. Repeating this 3 to 4 times per session retrains your brain to tolerate high arousal without triggering rapid climax. Reduce Friction Intensity: Use a generous amount of water-based lubricant and practice a light, relaxed grip. Evaluate Medical Options if Needed: If behavioral retraining and resolving the phimosis do not sufficiently delay ejaculation during partner intimacy in the future, a physician can prescribe short-acting, on-demand medications (such as Dapoxetine) or mild topical desensitizing sprays.
Health Tips
Never forcefully pull back a tight foreskin over the head of the penis if it resists. Forcing retraction can lead to paraphimosis, where the tight ring becomes trapped behind the glans, cutting off blood supply and creating a medical emergency. Separate Masturbation Speed from Real-World Partner Performance: Fast ejaculation during solo masturbation does not mean you will inevitably ejaculate rapidly during partnered sex. Partnered intimacy involves a completely different set of sensory, emotional, and physical dynamics. Let Go of Guilt: Masturbation is a healthy, natural biological function. Reframe your focus from "quitting" to building relaxed, intentional control over your physical responses.

Dear Patient, Thank you for sharing your concern in such detail. From what you have described, there appear to be two separate issues that need proper evaluation: 1. Phimosis (tight foreskin since childhood), which may require examination to determine its severity and whether treatment with medication, stretching exercises, or a minor procedure is appropriate. 2. Rapid ejaculation during masturbation. It is important to understand that the time taken to ejaculate during masturbation does not reliably predict sexual performance with a partner. Therefore, you should not judge yourself solely on the basis of masturbation. Masturbation itself is a common behavior and is not necessarily harmful when done in moderation. However, if it has become compulsive or is causing distress, difficulty controlling the habit, or affecting your daily life, it is worth addressing through behavioural strategies and, if required, professional guidance. Since you have also mentioned persistent phimosis and have been unable to reduce the habit despite trying yoga and exercise, I would recommend a detailed consultation. A physical examination can help determine the best treatment for your foreskin condition, and your ejaculation concern can be managed with a combination of counselling, behavioural techniques, pelvic floor exercises, and medication only if clinically indicated. Avoid self-medicating or purchasing over-the-counter products claiming to “cure” these issues without a proper diagnosis. With the right evaluation and treatment plan, both conditions are usually manageable, and most young men experience significant improvement. Dr. Fayyaz Sexologist & Men’s Health Specialist

Don't try to quit masturbation completely—reduce excessive frequency and avoid rushed masturbation. Get your phimosis evaluated. Premature ejaculation is treatable with behavioral techniques and, if needed, medications. Connect for discussion and treatment
