We are planning for kid from last 6 month and my wife's period missed 4 days back, test kit come positive. She has history of PCOS. What should we do now. Should we go for blood test now or wait. 3 month back it came positive but the after that she had her period.

For confirmation you should have a pregnancy test from a path lab rather than from a kit. If it comes positive then you should meet a gynaecologist.
