By looking at attached x ray it seems like there is inter dental carious, Which need to be evaluated properly. And their is no proper history. So consult with doctor online or offline for better guidance and treatment options.

Hi.. your x-ray is not very clear, but it looks like there is interdental caries in between the molar and premolar teeth. Both the teeth have to be treated.

The X ray shows a previous restoration on the tooth which is marked. Even attempting to redo the filling will lead to pulpal exposure, so this tooth (46) has to be treated with root canal followed by a crown.
