dear body has got healing powers but healing will be best if given assistance in the form of repair , so to avoid hearing loss you need to get it repair ,not all the perforations heal ,if you clean your ears than only recurrence is there
Next Steps
avoid self ear cleaning

Ear discharge more than few months called chronic ear disease. It requires medication for controlling of ear infection.  It decreases discharge first later perforation will be healed. For healing of perforation ear should be dry. For non healing perforation, persistant ear discharge requires early surgical intervention is required. Chances of ear complications will be there if it is not addressed properly.
