This is not looking normal. There is likely to be sweeling of glance penis, it seem to fungle / bacterial infection. You have to take hygiene & cleaning .Consult gen Physician.

It is swelling and there is an infection , it will require treatment with antibiotics and anti inflammatory medicines, also get your blood sugar checked .
