The symptoms described—mild burning during urination, redness around the urinary opening and possible irritation after vigorous/prone masturbation—can occur from local friction or minor urethral/meatal irritation. Vigorous masturbation is recognised as a possible cause of non-specific urethral irritation. However, burning urination can also occur with urethritis or a urinary infection, so it should not automatically be attributed to an old frenulum injury. The fact that he previously had a frenulum injury does not necessarily mean that the old injury has reopened or caused permanent damage.
Next Steps
He should avoid masturbation and any vigorous friction for several days and allow the area to settle. Since the burning has persisted for about a week and there is visible redness around the meatus, I would recommend an in-person examination rather than treating it based only on the appearance. If there has been any recent unprotected sexual exposure, urethritis/STI should also be considered; a clinician may appropriately evaluate the urine/urethral area depending on the history. Health Tips
Keep the genital area clean and dry and wash gently with water; avoid antiseptics, fragranced soaps and unnecessary creams. Avoid vigorous masturbation, especially using a bed/pillow or any technique that puts excessive pressure on the frenulum or glans. Maintain adequate hydration and do not repeatedly inspect or manipulate the urinary opening, as this can increase irritation. If he develops penile discharge, increasing pain/redness, fever, blood in urine, testicular pain/swelling or difficulty passing urine, he should seek prompt medical attention. Regarding his upcoming marriage: this type of irritation is usually manageable, and a previous frenulum injury does not by itself mean that he will have problems with intercourse or fertility. Thank you
