Patient is suffering from psychiatric illness, about which he is not aware. While his family members, who have noticed a change in his behavior, can consult a psychiatrist regarding the same. There are medications available that can be given by other means, if patient is not willing for the medicine currently. Also, he might need admission at this time. Detailed assessment by a psychiatrist is required.

His symptoms are self explained and can’t be objectively assessed unless he is examined by a psychiatrist. His conclusions are as he / she feels. If doesn’t want to see a psychiatrist exact diagnosis is difficult to make.
Next Steps
Must see a specialist.
