There is pain in right thumb and for every movement it sounds like kat-kat. All time it is paining..

This could be arthritis . Start with hot fomentation , ointment , exercises for the hand , antiinflammatory medicines.

There is a possibility of some arthritic changes in the joint which may be causing this noise..
Next Steps
I will suggest you to get one x ray of hand.. AP/oblique views. Also, start hot fomentation,  take analgesic tablet, you can also apply local analgesic ointment
