  4. Pain in right leg. Surgery is required?

Pain in right leg
Pain started two months back in right leg. Later on the right leg started lifting while walking and he started facing sever pain. We consulted neurosurgeon, he told to do MRI. What type of treatment and  surgery is required? Please find the MRI REPORT attached.
