Pain before nature call

Asked for Male, 39 Years

Below are the issues Before the nature call basically during early morning hours feel of a blockage in the rectum and pain during stool. Sometime feel of one vein is pulling inside the body. One current hits the testicles , it has happened 3/4 times. While in staircase stepping up,as if one vein pulls inside, I have to be in the same position for atleast 2 minutes to come back to the normal situation. Please advise...
