Ovary endometriotic cyst, bulky uterus

Asked for Female, 46 Years

My mom is having left ovary endometriotic cyst 32×24 mm internal echoes seen within the cyst. Is surgery required or not? And bulky uterus with multiple fibroids of size 32×26 mm seen. Identify the endometriotic posterial wall 24×14 mm(subserosal).
