Your symptoms â intermittent nasal block, ear popping, right-sided headache and eye socket pain, and cold sensitivity â suggest chronic sinusitis with possible Eustachian tube dysfunction. A deviated septum may be contributing. Since Fluticasone and Montelukast-Desloratadine haven't helped, this needs further investigation before deciding on surgery. Septoplasty and FESS (sinus surgery) are considered only after imaging confirms the extent of the problem.
Next Steps
Get an HRCT PNS (CT scan of sinuses) to assess septal deviation and sinus disease. Then consult an ENT surgeon who can review the scan and advise if surgery is warranted. In the meantime, try saline nasal rinses twice daily for relief. Consult me on Practo for further guidance.

Hello. Hope you are doing well.
Having seen your reports and read your symptoms, a sinus surgery will have to be justified by an in person examination ( nasal endoscopy). Also, a septoplasty is more justified as a treatment to improve the quality of life by relieving symptoms. So it’s essentially your call.
Next Steps
Kindly visit an ENT doctor.
Health Tips
Do steam inhalation and avoid extreme temperature changes. Take care.
Hello pa,
If your symptoms are not improving after the nasal sprays. We need to proceed with surgical management. But before that, a correct dose of the steroid nasal therapy for a correct duration should be done.
Kindly consult ENT specialist for further opinion
Hi
This report suggests of adenoid hypertrophy with mild sinusitis.
The adenoid hypertrophy could be a cause of the popping sound in the ear as it blocks the ear.
Also you have mild DNS as well.
Just the CT report does not warrant a surgery.
Need to correlate with nasal endoscopy.
Next Steps
to see ENT surgeon for nasal endoscopy
Impedance audiometry.
Consult with reports for further opinion.
