Nephew (3yrs old) have 103°F fever

Hello doctor, My nephew got 103°F fever now. He has fever since 4 days coming up-down. He is taking paracetamol syrup for this. He had some loose motions. Any urgent treatment or medications? Please suggest any. Anything worry about? Thank you.
