I am 23 yrs old Male 90kg weight and BMI : 30 currently on a Somatic Symptom Disorder Treatment and Health related Anxiety.
My physician recently started medication Escitalopram 5mg and Etizolam 0.25mg in the night and Petril MD 0.25mg SOS. It's my 13th day of the treatment and have experienced 2 episodes of panic attack within this treatment. Also, I am experiencing early awakes in my sleep after 4-5 hours and slightly increased anxiety after starting this treatment. My physician has asked me to continue this same treatment for 4 weeks and after 4 weeks he might increase the dose.
Do I need to increase my dose of escitalopram to 10mg OR any other change in medicine OR continue the same treatment?
Also, During the panic attack my BP spikes to 160+/100 and 90-100 Heart Rate, it causes headache and pressure during the attack in head. Whereas normally it range between 130-140/80-90 and 65-60 Heart Rate everyday.

Hello. Thank you for sharing your concerns so clearly - it's not uncommon to experience some fluctuations in symptoms during the early days of treatment for anxiety and somatic symptom disorders.
You're currently on Day 13 of treatment with Escitalopram 5 mg, Etizolam 0.25 mg, and Petril MD SOS, which is a standard starting regimen. It's important to understand the following:
1. Initial Worsening of Anxiety:
Escitalopram and other SSRIs can initially increase anxiety, restlessness, or sleep disturbances in some individuals. These effects usually subside in 2–4 weeks as your body adjusts.
2. Time to Full Effect:
Escitalopram often takes 3–6 weeks to reach full therapeutic benefit. Early awakening and intermittent panic episodes at this stage don't necessarily mean the treatment is failing.
3. Dose Adjustment:
Your physician’s plan to reassess after 4 weeks is appropriate. The usual therapeutic dose of Escitalopram is 10–20 mg/day, but starting low and going slow helps minimize side effects.
4. Etizolam and Clonazepam (Petril MD):
These are short-term agents to help manage acute symptoms like panic. Over-reliance should be avoided due to dependence risks.
5. BP and Heart Rate Spikes During Panic:
Sudden spikes in BP and HR during panic attacks are typical due to the fight-or-flight response. Headaches and discomfort during such episodes are common but usually not dangerous in a healthy young adult.
Next Steps
a. Continue the current treatment as advised by your doctor unless the symptoms worsen significantly.
b. Avoid self-adjusting doses. Increasing Escitalopram without supervision may lead to more side effects.
c. Consider discussing with your doctor the possible use of non-pharmacological supports, such as CBT (Cognitive Behavioral Therapy), which is highly effective for panic disorder and health anxiety.
Lifestyle measures like regular sleep, avoiding caffeine/stimulants, exercise, and mindfulness can also support recovery.

You're currently on Day 13 of treatment with Escitalopram 5 mg, which is a starting dose. It’s common to experience slight worsening of anxiety or early morning awakenings in the first 2–3 weeks as your body adjusts. Panic episodes and transient BP spikes during attacks are typical in anxiety and not dangerous if your baseline BP is stable. Since your physician has planned a 4 week reassessment, it’s best to continue the current dose unless symptoms worsen significantly. Escitalopram often shows full benefit after 4–6 weeks, and a dose increase to 10 mg may be appropriate at that point if needed. You’re on the right track just keep regular follow ups and maintain healthy lifestyle habits alongside medication.
Hi … Currently You are on ur 13 th day of treatment usually SSRI medications take time to act so the response will be slow and steady mostly can be seen after 7-10 days. As you are experiencing early morning awakening it’s better to consult a psychiatrist for proper evaluation.

Proven technique for Anxiety and Panic episodes
Hello. So basically escitalopram (or any other antidepressant class of medications) take a couple of weeks to reach their maximum affect, and that is why your psychiatrist has asked to wait for a few weeks. Secondly, these medications can increase anxiety in some patients in the initial days of therapy, which is their normal side affect and usually becomes better by week 2 of treatment.
Next Steps
Can consult your psychiatrist to start some low dose anxiolytics for the time being.
