Its cancer that has spread to neck .. should be seen by oncologist/ ent onco surgeon specialist for further examination and investigations( biopsy, pet-ct/ct/mri etc) .
Then treatment will be decided. she is very elderly . Oncologist / surgeon will decide whether she is fit for any treatment.

Its likely a cancer from mouth or throat that has spread to the neck.
Please get a complete ENT examination from ENT or Head and Neck Oncosurgeon for knowing the actual area where cancer started to know the type and stage of cancer.
Since 84 year old, she needs to assesed if she will fit to receive any form /which form of treatment.
