Most likely it is koch's pathology. Kindly have it cofirmed by lymph node FNAC.  Then Start ATT.

Looks like a chronic inflammatory condition of L3-L4, mostly tuberculosis. Especially when associated with fever, chronic cough, weight loss and night sweats .

Psoas abscess indicates presenc of a tubercular infection most likely. Get it drained ASAP. Send the pus for culture and staining. Start ATT
