"Multiple enlarged pretracheal lymphnodes are seen with cetral non-enhancing areas with suggestive of necrosis largest measures 21 x 11 mm."

Needs detailed past history. Lymphadenopathy could be tubercular, neoplastic or calcified. What made you to for ct thorax? Without assessment conclusion couldn't be drawn. Better to get physical consultation.
