Hello. Need CT guided biopsy from the lung lesion to confirm cancer. Is he smoker or non smoker Biopsy confirms whether it’s squamous or adenocarcinoma type. If he is smoker then squamous type is more common. If it’s adenocarcinoma then further Immunohistochemistry is required to know the EGFR, ALK, ROS1, PDL1 status based on that whether chemotherapy or immunotherapy depends.

It seems to be case of ca lung with nodal metastasis. Cect or USB guided biopsy from the lung lesion along with Immunohistochemistry . PETCT is required to rule out distant metastasis

Please urgently get a CT Guided biopsy as Dr Lalit described. These lesions can sometimes be very aggressive and need timely management. Based on pathology reports and additional special tests, treatment will be planned.
