The picture you have attached does not look like a fungal infection. You seem to be having pitting of the nails. Pitting is usually associated with some problem in the nail matrix (the area under the skin from where the nail grows). Pitting occurs in diseases like alopecia areata (patchy hairloss), psoriasis, lichen planus, nail dystrophy or nail matrix eczemas. However a picture is never as good as examination in person. The medicines you have been given are the right medicines if it is a fungal infection. Fungal infections of the finger nails take 3 to 6 months to respond and those of toe nails take 6 to 9 months. Fungal infection will start responding to treatment after 1 month of antifungal medicines. Pitting does not respond to this treatment and the underlying cause must be found out. If it is nail dystrophy then it will not respond to any medicines and may eventually cause complete destruction of your nails. I suggest you get a second opinion or ask your existing dermatologist regarding the diagnosis if you do not notice improvement even after 2 months of treatment with antifungals. Hope this helps you. Warm regards. Dr. Rajdeep Mysore. Chirag Hospital Bangalore
